Please find attached the Basketball Australia personal injury claim form.
All insurance claims in Victoria will now first be signed by the association to confirm registration as per normal, but will now go directly to the Insurer PSC HORSELL address.

NOTE: Can all members putting in a claim form please note that Section B, the 'Association Declaration Section' must be filled out by your association before sending to Horsell.
If this is not done the claim will not be processed as they will not believe the player to be registered for insurance.
